Exhaust wont help with gas mileage, just make it louder, unless you went the expensive route and bought long tube headers, high-flow cats.
CAI might add 1 or 2 mpg not sure though. Gotts mod is cheaper and same outcome if your interested in that.
Programmer with the right custom tunes will deffinetly help out on mpg. Also let them know you have a CAI (if you dont do the Gotts) so they can make the proper changes to your custom tune.

mileage depends on the truck. i had a 04 f150 heritage with a 4.2L...reg cab longbed 2wd 3.08 rearend. got 17.9mpgs combined driving stock...K&N drop-in i got 18.9 and with the K&N intake i bought i got a pretty consistant 21.5-22.5mpg combined. i put the same intake on my 08 when i traded in the 04.. The 08 is a reg cab shortbed 2wd 4.2L with a 3.55 rearend. both were 5spd manuals too btw. the 08 weighs about 450Lbs more...is more boxy, higher off the ground, higher rpms at speed, and driven the same way. i went from about 19mpg stock to 20mpg after the intake. all of this in Pennsylvania...i moved to Colorado and bought a tuner with custom tunes from VMP. have been running the 87 performance tune on 87 octane and my last 2 tanks were 16.9mpgs and 18.9mpgs(babying it). I do have a heavy foot also btw. Also in Colorado everyone tells me i should get better mileage because of the thinner air....well yes its thinner...but its also blowing at like 20mph-30mph all the damn time so it is less now than i got in PA. i would say dont expect a whole lot from the 5.4 if you add the mods you are thinking about. i would say if you did exhaust, tune and gotts or CAI, you MIGHT see +1-2mpg. of course that depends on your gears, heavy or light foot, lifted, bigger tires...etc

yeah the GOTTS mod is basically a higher flow intake run from the same area as your stock intake. it has a bigger opening than stock so it can take in more air like a Cold Air Intake but it is about $10 compared to the $305 i paid for my K&N intake. and it is completely reversable. it is better for the V8s because a lot of trucks are running lean from the factory. adding a cold air intake changes the location of the mass airflow sensor which makes it read even more lean. the GOTTS uses the stock sensor location so it doesnt do anything to the lean condition.
