There are a couple of companies that custom builds those.
They basically just use an F650 Crew Cab blank chassis and throw on an F350 dually bed.
The really cool thing is it can be had from the factory with a Caterpillar diesel and Allison auto transmission! How's 860lb-ft @ 1440rpm sound!!
