OBS F-250/350 weighs about 2,000 lbs less than a similar 99-12 truck

What's your boost pressure max out at when you get on the truck? When the cat plugged up on my truck I would only see 19-21 PSI of boost and that will absolutely kill your mileage because the truck can't breath.
maybe 10-12 PSI boost. and the sticker on the pump says something something 5.0 biodiesel..

You have a either a fuel problem, turbo vanes are seized, or you have a cat that's plugged solid. The boost should be maxing out at 25-28 psi.

check the tail pipe and see if it is loud and flowing, then i would pray it is just the VGT acting up and not seized.

I'm sorry I misread, according to CTS boost maxes at 23 psi at wot

It's not loud, it's all stock. When I turn the truck off you can hear the turbo spooling down and it has like a, bad bearing sound, like a semi grinding sound I guess. I'll see if I can make a video

sounds like VGT system is wonky in some way.

unplug the VGT solenoid on the turbo and take it for a ride. does it do anything different?

Not even comparable with weighing a ton less and way less power made.

I'll check it out when I get off. How much would that be to have fixed?

it would be pretty cheap if you did it. just pull the turbo and disassemble and clean the unison ring for the VGT system. it most likely is rusted up in one position and cant move to let your boost build correctly.

your making some boost so it isn't stuck open. the VGT actuator solenoid could be bad too. that would be more money to fix.

do you have a tuner that can monitor the engine and VGT duty cycle and stuff?

That's still on the low side but won't be enough to cause the PCM to trigger the CEL. Mixedbreed is most likely correct and the vanes are just moving so far and stopping due to rust scale or gummed up with soot from to much idling or in town driving and not getting the EGT's hot enough to burn all that crap out. When I was driving my truck at least twice a week I would purposely push the EGT's up and past 1100* for very short burst just clean the turbo out. Plus it's fun just to stretch the old girls legs a little.

What are the EGT's when you are cruising at speed? They should be around 600*-700* just cruising and moderate acceleration you should probably see no more than 800*-900* with the EGT's hovering around 400*-450* just idling.

I have an edge insight CTS but I just installed it so I haven't really set it all up. And I haven't ran my EGT probe yet I will do that tomorrow

true the EGT's will skyrocket in a few seconds if not monitored. but sometimes that is good for the truck.

could be the EGR valve too now that i am thinking about it. stuck open and just letting it flood the intake with hot spent exhaust...

so many things it could be too

fuel filters clogged, air filter clogged, VGT system, EGR system, check the basics first, then worry about the rest. EGR valve is Basic compared to the turbo.
