You don't technically have to change gears. I know somebody who runs 38.5s on his f150 with stock gears. It's mostly about what kind of driving you do and what kind of performance you want out of your truck.
If you want your truck to stay close to stock, I wouldn't go bigger than 33s. With a 5.4 you could turn 35s, but your city mileage is gonna suck and your once-quick truck will be a dog.

The difference between 35s and 38s is pretty big. 35s and 4.56s work very well together, and 38s with 4.88s would be a good combo. If you really plan on going to 38s then regear to 4.88s. But don't do that until you have at least 35s on there. If you're not going over 37s then leave it at 4.56s.
