could be several things. u-joints are typical. u replaced front and rear? should be golden there.
next thing, seems overlooked sometimes is the bolts to the companion flange. no loctite on them and they tend to work themselves loose.
or your slipyoke has too much play in the splines, should probably be replaced before it wears out the rear output shaft.
