If I wanted to go with 37's would I have to regear it at the same time. If I do not regear it does it just have a lack of power?

You would want to regear with 37's. There would be a big loss of power, plus the added stress on your transmission.

regearing isn't required, but it will get most of your power back in the proper rpm range. josiah has had 37's and 3.73's for a while now.....

I have the 37" Pro Comp Xtreme All terrains running on my truck and everythings fine. If you do a sharp turn to the right or left and hit a decent size bump I rub a little but other then that it never hits. And as far as regearing if you dont have the money right now you should be ok the truck still runs good. If my camera wasnt broken I could get you some pics but it does look good when you pull up to a F350 with a lift rollin on 35s and you got bigger rubber then them .

BTW if anyone knows of somebody looking for a lifted 2004 F150 XLT with 6" FABTECH coilover and the rubber above and some extras let me know or its in the classifieds, I'm asking $15,000 but very willing to talk.

You will want to re-gear with 35" tires too.

There are many good ones but I would recommend Yukon.
