Check engine codes P2198 and P0176

I have a 2005 F150 5.4 FX4. On Saturday it started idling rough and check engine light came on. Had it check stated misfire. I replaced all 8 plugs and wires. On Sunday light came on again stated it was running rich. Took MAF out and cleaned it. Same issues, went ahead and just replaced the MAF. Today, Monday, filled tank and added B12 and light came on again. Running rich Bank 2 sensor 1 bad and running rich. Replaced the O2 sensor. It has started running better still alittle rough idle but nothing compared to what it was and got two more codes P2198 and P0176. Im at a loss as where to go from here. Any help would be greatly appreciated!

Make sure the correct sensor was replaced. The third post here has a figure with some ideas where to look:

Ended up having a fuel injector stuck open also. It was a perfect storm of everything going wrong at once. But got everything fixed and it runs like a champ now. Thanks for the input!
