could be a number of things but most likely things are dirty/rusted caliper slides causing hang ups, warped rotor, dirty brake pads, air in fluid.. Best thing to do is to remove it all, service the caliper slides and grease them, spray brake kleen all over pads/rotors and make sure they are clean and take it from there. Chances are the rotor needs to be replaced though.

The swishing sound is the pads and the rotar making contact as the wheels spins. It clears then hits clears then hit etc. Chances are you have a rotor that is slightly warped, depending on how bad it is and how much use you have gotten out of them you may want to repair OR replace them.

When you brake hard I am willing to bet you feel a shimmy/vibration?

First thing I would do is clean it all out. If that dosent work I would go with new calipers.
