If the centerbore of the wheel is larger than the OD of the hub, and you don't have rings in them, I'll bet my lunch money that it's your problem. If the nuts are a flat-seat nut, I'll guarantee it. It's near impossible to get a good center on them without rings and relying just on the taper of the nut to provide a "lugcentric" fit.
Get some rings ASAP.
